How can I get a copy of an EMS patient report?
To obtain a copy of an EMS patient report, the actual patient or another person with a written medical authorization signed by the patient must request the report through the Fire Administration office. Whether you are the patient or an authorized representative (as described above), you must bring proper identification. Due to the medical information contained in these reports, we are prohibited by law from releasing them without the patient's written authorization.

EMS patient reports are available in the Fire Administration offices, located at 1515 N. Galloway, Mesquite, TX 75149. Office hours are Monday through Friday from 8:00 AM to 5:00 PM.
