If you have been directed to this site because you have scanned the QR Code on the Accident Information Exchange Form – please note that this form is not a crash report (DPS Crash Report CR3). This form (Information Exchange Form) was designed to expedite the minimum amount of personal information necessary for each driver to comply with the state law. You, or your insurance agent, may obtain a copy of the call log (Call Sheet) from records. There will not be a DPS Crash Report (CR-3) associated with this incident if the Information Exchange Form has been given to you.
